Is there a hack or a module i can use with gallery so that when someone uploads a picture they will have an option to list their contact information such as "email", address ETC before they upload the picture etc.

the newest version allows, during setup, to specify additional optional fields that users can fill in while uploading photos. grab the latest CVS version and give it a shot.
